eAdmissions portal

Pre-admission is an important part of your day surgery care. Completing the Admission Form electronically via our eAdmissions Portal will ensure we can confirm your admission, financial and other arrangements in an effective and efficient manner.

If you are unable to complete forms online, in the first instance, please ask a family member, carer or friend to help with this part of the process.

We understand how sensitive personal and health information is, so we take certain steps to ensure that electronic data entered is always safe. The same encryption process used for internet banking is applied and data is protected by strict Australian regulations.

We ask that:

You complete ALL the questions provided via the eAdmissions portal
Submit the eAdmission Form no less than 14 days prior to your admission to Vista Day Surgery
Your doctor completes the Consent forms

To assist you with this process, please have the following information at hand:

Date of admission
Admitting doctor name
Details of person collecting and staying with you after discharge
Medicare details
Health Fund details (e.g. DVA, Private Health Insurance etc.)
Current medications taken

To complete the Admission Form via eAdmissions, you will need a valid email address to create a Login account.

Please select eAdmissions Portal link to complete admission process electronically.

Once you have created a Login, you can:

Start the admission process by competing the form
Save form for later edit
Submit completed Admission form securely
Download or print your completed form for your records
If you’re returning for a subsequent surgery, simply log in to your existing account and your contact details will already be saved.

Using the eAdmissions Portal

First admission at Vista Day Surgery

The eAdmissions Portal Login link will give you an option to:

Create Account

For your first admission, click the '+Create Account' option.

You will need to provide an email address and a password to confirm your account and to be able to submit the forms.

You are able to start your forms, save and submit your admission using your account.

Make a note of your login details (email and password) for future admissions so you do not have to repeat the entire process again.

You will receive a SMS to confirm that we have received your forms once assessed by a staff member.

Second/subsequent admission at Vista Day Surgery

On the eAdmissions Portal Login page, select the option:

Enter your login details (email and password)

Click on the three small dots in your previous admission field

Select 'Re-Use Admission'
Select 'Next'

From here you can check the details you provided previously and make any changes if relevant i.e: Date of admission, a new medication, a different person collecting you on discharge.

Submit

You will receive a SMS to confirm that we have received your forms once assessed by a staff member.

How to reset password

In the event you have forgotten your password, you can reset it by following these steps:

In the eAdmissions Portal Login page:

Select Create account
Select the 'Forgot password?' link to reset the password

Pre-admission

7-14 days prior to admission

Once we receive your completed admission forms, they will be reviewed by an Admissions Nurse and you will receive a confirmation text message to your nominated mobile phone number. The message will provide further information about your upcoming admission.

A nurse will contact you should any additional information be required.

1-4 days prior to admission

You will receive another text message informing you of your admission and fasting times once these have been confirmed with your surgeon’s rooms.

The message will also confirm your Health fund cover and outline any relevant out of pocket expenses. All excesses attached to your hospital fund are paid on the day of surgery and are an out of pocket fee (not rebatable).

Submitting your Admission Form online supports Vista Day Surgery's electronic medical records processes and we appreciate your support in utilising this format.
