Pre-admission is an important part of your day surgery care. Completing the Admission Form electronically via our eAdmissions Portal will ensure we can confirm your admission, financial and other arrangements in an effective and efficient manner.
If you are unable to complete forms online, in the first instance, please ask a family member, carer or friend to help with this part of the process.
We understand how sensitive personal and health information is, so we take certain steps to ensure that electronic data entered is always safe. The same encryption process used for internet banking is applied and data is protected by strict Australian regulations.

To complete the Admission Form via eAdmissions, you will need a valid email address to create a Login account.

For your first admission, click the '+Create Account' option.
You will need to provide an email address and a password to confirm your account and to be able to submit the forms.
You are able to start your forms, save and submit your admission using your account.
Make a note of your login details (email and password) for future admissions so you do not have to repeat the entire process again.
You will receive a SMS to confirm that we have received your forms once assessed by a staff member.
On the eAdmissions Portal Login page, select the option:
Enter your login details (email and password)
Click on the three small dots in your previous admission field
From here you can check the details you provided previously and make any changes if relevant i.e: Date of admission, a new medication, a different person collecting you on discharge.

7-14 days prior to admission
Once we receive your completed admission forms, they will be reviewed by an Admissions Nurse and you will receive a confirmation text message to your nominated mobile phone number. The message will provide further information about your upcoming admission.
A nurse will contact you should any additional information be required.
1-4 days prior to admission
You will receive another text message informing you of your admission and fasting times once these have been confirmed with your surgeon’s rooms.
The message will also confirm your Health fund cover and outline any relevant out of pocket expenses. All excesses attached to your hospital fund are paid on the day of surgery and are an out of pocket fee (not rebatable).
